Vision:The
tri-county region will be recognized as a “high
performance
health and wellness community” characterized
by steady and measurable improvement in the
health and wellness of its citizens and their
access to quality medical care.
Community
Outcomes:
• All residents in the Trident Area will take
personal responsibility for maintaining and
improving their mental and physical well-being.
• All residents in the Trident Area will have
access to continuing education and information
about how to maintain and improve their mental
and physical well-being and how to best utilize
the health care system.
• All residents in the Trident Area will have
a medical home and access to at least the minimum
level of medical and preventive care needed
to maintain reasonably good health.
Strategic
Direction:
• Put United Way in a position of community
leadership on issues of promoting health and
wellness, not by undertaking all these tasks
ourselves, but by serving as a catalyst to help
other institutions such as schools, employers,
churches and nonprofits integrate healthy living
messages and activities into their own agendas,
programs and policies.
• Choose measurable indicators of progress toward
achieving each of the Health and Wellness Community
Outcomes. Choose indicators based on baseline
data available in the Tri County Health Indices
Survey, SC Data Warehouse and TUW's Asset Mapping
Tool. Focus on adding value to programs that
have an effect on the chosen indicators.
• Use TUW's policy protocol to recommend appropriate
levels of action around pending legislation
that would further our Health and Wellness Community
Outcomes.
